Calcium folinate Usage

Calcium folinate (CF) itself has no anti-tumor effect and is mainly used for high-dose rescue and concurrent use with fluorouracil to enhance the latter’s therapeutic effect.

The so-called high-dose methotrexate-leucovorin (HD MTX-CF) rescue therapy is applying intravenous infusion of MTX of over 100 times higher than the conventional dose, usually at 2~5 mg/m2 for drip of 4~6 h to make the blood concentration increase to a very high level, so that intracellular MTX can reach over 10-5 mol/L or more effective concentration.

In this case, it can be spread to solid tumors with poor blood supply, and reach across physiological barriers such as blood-cerebrospinal fluid and testicular.

However, high-dose of MTX can have a fatal toxicity, so at a certain period of time after the MTX dripping, we must immediately take detoxification measures.

CF rescue should generally start at 2 to 18 hours after the MTX dripping. Apply intramuscular injection or intravenous injection of a dose of 6~15μg/m2 at 6 hour per time with the general medication of 3d and a total of 12 times.

CF dose and injection time should be associated with the application of MTX, namely being able to reduce the blood concentration of MTX to below the safety threshold: 10-7 mol/L before stopping. In addition, we should also give auxiliary hydration, alkalization treatment to support.

CF formulations are generally 6 mg water injection.

For the treatment of megaloblastic anemia, 15 mg orally every day; or 1~5 mg intramuscular injection every day. After 10-15 days, the dose can be reduced to 5 mg/d orally until the blood picture is normal and the symptoms disappear; if adenosylcobalamin cannot be excluded If it is lacking, it should be combined with adenosylcobalamin.

For the rescue of methotrexate, 3 to 4 times a day, 5 to 15 mg orally each time for 2 days; or 3 to 6 mg intramuscular injection; methotrexate taken for more than 4 hours is invalid with this medicine.

For the detoxification of pyrimethamine or trimethoprim, 5~15 mg orally every day. Calcium folinate is used in combination with fluorouracil, 20-30mg/m2, orally half an hour after fluorouracil is administered.

Calcium folinate use

1. Mainly used as an antidote for folic acid antagonists (such as methotrexate, pyrimethamine or trimethoprim, etc.).
2. It is used to prevent severe toxic effects caused by methotrexate overdose or high-dose treatment. 3. Megaloblastic anemia caused by folic acid deficiency.
4. When combined with fluorouracil, it is used to treat advanced colon cancer and rectal cancer.

It is used to relieve the toxic reaction caused by the overdose of aminopterin and methotrexate, and can also be used to treat giant red blood cell anemia.

Calcium folinate counteracts the effect of folic acid antagonists, which are combined with folic acid dehydrogenase (dihydrofolatereductase) to prevent the conversion of folic acid to tetrahydrofolate. Medically, after methotrexate chemotherapy, leucovorin is used to reduce the toxicity of methotrexate. Leucovorin also enhances the cytotoxic effect of 5-fluorouracil in anti-cancer.

Leucovorin can counteract the effects of folic acid antagonists. Folic acid works by binding dihydrofolate reductase and blocking the conversion of folic acid to tetrahydrofolate. In medicine, leucovorin is used to reduce the toxicity of methotrexate after methotrexate treatment.
