What’s the application of Lopinavir?

• The oral bioavailability of Aluviran is poor due to the high expression of P-GP and MRP2-mediated efflux pump on the luminal surface of intestinal epithelial cells, the extensive metabolism of CYP3A4 enzymes in the liver, and the low water solubility of Aluviran itself (40 mg/mL). The efflux pump action is associated with the cellular excretion of foreign substances and toxic molecules, so the strategy to improve the bioavailability of Aluviran is focused on inhibiting the normal physiological function of the efflux pump, there is a risk of serious side effects to the organism. Ritonavir is not only a protease inhibitor but also a strong inhibitor of CYP450 enzymes (CYP3A4 is its isoenzyme). Using small doses of ritonavir to competitively bind CYP3A4 with Aluviran can slow down the metabolism of Aluviran, improve the bioavailability of Aluviran, and enhance clinical efficacy. Abbott had developed a combination formulation of Aluviran and ritonavir consisting of 1 unit dose of Aluviran and ¼ dose of ritonavir. The formulation was approved by the FDA for marketing in the United States in 2000 and China in 2007 under the trade name Kaletra and is primarily used clinically for the treatment of AIDS. UsesAluviran is an HIV protease inhibitor and antiretroviral drug. It is used for the treatment of HIV-1 infection in adults and pediatric patients over 6 months of age.
