What is AtracuriuM Besylate?

• The official answer：Atracurium is a moderately chronotropic non-depolarizing muscle relaxant and is commonly used as its benzenesulfonate salt. It is mainly eliminated by non-enzymatic degradation, but can also be hydrolyzed by pseudocholinesterase into inactive metabolites, which are then excreted through the kidneys and bile. The effect is weaker than that of cartridge poisoning, but the onset of action is rapid (1 min), the duration is short (15 min), there is no accumulation of repeated administration, the effect on ganglion blockade is light, and there is no effect on heart rate. It is used clinically for various surgical procedures where inotropic relaxation is required to facilitate controlled respiration administered by sedation, with a starting dose of 0.3-0.6 mg/kg, and then can be maintained by sedation at 5-10 μg/kg per minute. Soluble in water, very soluble in ethanol, acetonitrile and dichloromethane. It contains three isomers: cis-cis, cis-trans and trans-trans.

What’s the application of AtracuriuM Besylate?

• Atracurium is a synthetic bisquaternium ester-type benzylisoquinoline compound, which is a non-depolarizing muscarinic agent of the medium-latency barrel toxicity class. It can act mainly through competitive binding to cholinergic receptors and can be reversed by anticholinesterase drugs such as neostigmine. Rapid onset of action, good muscle relaxation effect. The effect of 0.4~0.5mg/kg can be seen in 1~2 minutes with peak effect in 3~5 minutes, and the maintenance time is 15~30 minutes. It can be degraded by Hofmann (i.e. by non-specific esterase degradation and base hydrolysis) to inactive metabolites at physiological temperature and pH environment. The plasma protein binding rate is about 80%. Mainly excreted as a metabolite via urine and bile with an elimination half-life of approximately 20 minutes. May cross the placenta in small amounts. It is used for skeletal muscle relaxation in various surgical anesthesia to facilitate controlled breathing, especially for maintaining muscle relaxation during tracheal intubation and cesarean delivery, and can also be continuously infused to maintain a prolonged neuromuscular blockade. It is suitable for patients with hepatic and renal insufficiency, patients with jaundice, pheochromocytoma patients for surgery, and patients for outpatient surgery.
